Information needed to port out to a new carrier

Hello:
I need my account and pin # associated with one of the phones on my account, to port out to a new carrier. How do I get that information?

It is also the last 15 digits of your SIM card if your phone is activated with the GSM BYOP Program. We can also check on it for you.
