Having trouble activating new phone

I am a current customer trying to activate a new phone. I added the phone to my account but cannot activate it (getting errors). I want to keep my old phone number and minutes. I bought the phone through Amazon which included $30 of airtime. I currently have 690 MB data, 931 minutes and 1039 text.

Please assist.
